 NEW! Documentation Checklist 

A new Documentation Checklist has been added to the online Benefit Coordinator Reference Manual. It can be found on page 9 in the Forms and Instructions section.

This Checklist will assist you in knowing what documentation is needed, especially when (as sometimes happens) documentation could not be submitted with the Enrollment or Change-in-Status form. A completed Documentation Checklist will also speed the processing of the information by PEIA by matching the documentation to the policyholder. Additional information can be found in the new online Benefit Coordinator Reference Manual.
