Benefit Coordinators no longer need to request the Evidence of Insurability form when an employee submits an application to Minnesota Life. Minnesota Life will now request an EOI directly from the employee if one is needed. The employee will receive an EOI packet which will include a user id and password to access Minnesota Life's website. The employee can then complete the EOI online using real-time underwriting response. If the employee does not wish to use the online option, an EOI form will be included in the packet as well so that the employee can complete and return it by mail to Minnesota Life. Please remember that as a benefit coordinator you only need to submit the application to PEIA now.
