Policyholders covered by a Preferred Provider (PPB) Plan who have lost their ID card or did not receive a new ID card within a few weeks of enrollment should contact HealthSmart at 1-304-353-7820 or 1-888-440-7342 (toll-free). Policyholders can also go online to www.HealthSmart.com to print out a temporary ID card. Once at their website, click on the Members tab near the bottom of the page. When prompted, click on Well Fargo Customers. If you are not already registered, simply click on the "Click here to register" link. Enter your information, including your Member ID (available from your Benefit Coordinator); the Group ID is 7770. Once you have completed registering, there will be a link in the left panel to print a temporary ID card.
