When it becomes available, the vaccine for the H1N1 (swine flu) virus will be available to members under Preferred Provider Benefit (PPB) Plan A or B. The federal government pays for the vaccine itself, and PEIA pays for the administration of it.
Before you get the H1N1 vaccine, ask your provider if he will bill PEIA. If the provider will not, you may either go to another provider or pay for it yourself and submit a claim to Wells Fargo for reimbursement. Keep in mind Wells Fargo will only reimburse you for the cost of administering the vaccine, not the cost of the vaccine itself.
Here is the form to submit to Wells Fargo:

Standard flu shots also are covered in full by the Preferred Provider Beneift (PPB) Plan A or B.
