If you would like access to explanations of benefits (EOBs) and other health information online, register your account at
www.healthsmart.com. The process is easy, and it will make tracking your benefits easier.
To register, you'll need your ID number (located on your ID card) or social security number, and your group number (also on your ID card). You also will need the policyholder’s first and last name, date of birth and zip code.
To help safeguard your health information, the Web site will prompt you to create a user name and password. Be sure to read the requirements before selecting them, and keep this information in a secure place.
