Renal Care Management Program 


A renal care management program is available to all PEIA PPB Plan primary members with end-stage renal disease (ESRD) or chronic kidney disease (CKD). The third party administrator Renal Nurse Care Manager, Nancy Sigmon, communicates with the member, their dialysis facility, if applicable, as well as the treating physicians to coordinate the best possible care for the member.

The Renal Care Management Program includes:

  • Kidney health educational materials, such as the National Kidney Foundation's booklet "Are You at Risk for Chronic Kidney Disease?"
  • A visit by the Renal Nurse, as appropriate, at the member's home or dialysis center to determine clinical and/or psychosocial needs
  • Review of medications and lab work with the member
  • Education on the different dialysis access types
  • Education regarding the transplant process through LifeTrac, PEIA's transplant network
In addition, if the member requests and scheduling permits, the Renal Nurse will go with the member to their visit with a kidney specialist.
